One of the most popular house rules that I've used over the last 4 years or so is a chart for Dynamic Critical Hits. It was born in the first days of 4th edition, when I was missing the old school, anything-can-happen, narrative-driven combat of my second edition days.

Sowm and the Creation

First there was an ocean, a vast concourse of water that grew and stretched the bounds of all existence. The jharethil call it Norahaltath, which by interpretation is the Sea in the Void. There was no air, no land, no life; just the endless churning of a drowned universe.
